The following list is the License/Permits that are issued by the Illinois Department of Labor.

• Day Labor Services Agency Application Form 
• Nurse Agency License/Renewal
• ODRISA Permit Application 
• Private Employment Agency License
• Private Employment Counselor License​
• Amusement Ride & Attraction Operation Permit

Note:

• If you are experiencing problems accessing the Application, please try using Internet Explorer or if you use Chrome, right click on the link, download the Application, and select "Save link As", it will save the pdf to your computer, then open it up with Adobe Reader.
• Once you have saved the document to a local hard drive, re-open and fill out all of the application, save, and Print and submit to the Department of Labor.
